In the spirit of a collaborative effort:

Northeast Florida Counts is a FREE online health resource for the seven county area: Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia. The site helps planners, policy makers, and community members learn about issues and identify improvements necessary for healthful outcomes. Funded by a broad partnership and managed by the Health Planning Council, Northeast Florida Counts illustrates a regional commitment to delivering a shared community resource.

This community-driven, dynamic, and interactive web-based information system provides easy to understand health, environmental and other sustainability indicators for Northeast Florida. This innovative website also links its visitors to promising practices and resources that encourage and promote informed community action. The site addresses the issue of accessibility to data and serves as a road map for collaborative community improvement by highlighting where we have been, our current status, and directing where we need to go.

Northeast Florida Counts Steering Committee

The Steering Committee is the “working” group charged with designing and selecting indicators for Northeast Florida Counts, as well as reviewing applications and selecting recipients for the Northeast Florida Counts Student Scholarships.

Elected Steering Committee Members

Additional regional partners can be elected to serve as an honorary member of the NE FL Counts Steering Committee.

Northeast Florida Counts Advisory Committee

All regional partners have the opportunity to serve as members of the Northeast Florida Counts Advisory Committee.  Under the direction of the Northeast Florida Counts Editor & Community Relations Coordinator, the Advisory Committee will be charged with developing and managing the website content, developing content submission policies, and developing reports for the Steering Committee.

The Health Planning Council of Northeast Florida

The Health Planning Council of Northeast Florida is a part of a statewide network of 11 non-profit agencies established by Section 408.033 Florida Statutes (F.S.) to conduct regional health planning and implementation activities. The Health Planning Council provides four core services: Community Health Planning & Assessments, Healthcare Utilization Studies, Community Convening; and Fiscal & Administrative support services for non-profits and coalitions.

The Health Planning Council of Northeast Florida provides coordinated health planning services and non-profit support designed to promote population health and to enhance the provision of accessible, affordable and high-quality healthcare services to everyone in Northeast Florida.
