- St. Johns County Health Leadership Council
The St. Johns County Health Leadership Council is a county-wide health assessment and planning Council, championed by the Florida Department of Health - St. Johns County Health Department.
- Sports Concussion Task Force for Volusia and Flagler Counties
The topic of sports concussion is very important in local clinical practices and is a priority for the local sports medicine community. Upon the recommendation of the Health Planning Council and area clinicians, Volusia and Flagler Counties are moving forward with a community-wide Sports Concussion Task Force.
- Discovering Brentwood
The Health Planning Council of Northeast Florida, in partnership with the War on Poverty and the City of Jacksonville Planning and Development Department, is undertaking a Healthy Places Initiative for the Brentwood Community.
- Northeast Florida Cancer Control Collaborative
The Northeast Florida Cancer Control Collaborative (NEFCCC) is a network of stakeholders committed to cancer prevention and treatment. The NEFCCC works to achieve the goals and strategies of the 2010 Florida Cancer Plan in coordination with the Comprehensive Cancer Control Program.
- St. Johns River Rural Health Network
Rural Health Network works under a mission to develop, encourage, and support partnerships and initiatives that improve the health of rural communities in Baker, Clay, Flagler, Nassau, St. Johns, Putnam, and Volusia Counties. Most importantly, Rural Health Network focuses on the specific health issues that impact people who live here in our rural areas of Northeast Florida; and serves as a primary platform to align and integrate local public and private resources around those issues within a culture of real partnership and collaboration.
- Healthy Places Initiative
The Healthy Places Initiative advances public health and quality-of-life in built environment planning and policy to ensure that all neighborhoods in Northeast Florida promote and provide the opportunity to live a healthy life.
- Assessing the Needs of Elders
The Health Planning Council of Northeast Florida is conducting the Elder Services Needs Assessment for the seven-county area served by ElderSource.
- Duval County Immunization Task Force
The mission of the Task Force is to develop and implement community wide campaigns that will improve immunization rates for children and adults living in Duval County.
- Bicycle and Pedestrian Safety
Northeast Florida is similar to many places in the rest of the county who are addressing bicycle and pedestrian safety. Creating safe and healthy communities by focusing on the environment and how people behave in space is not a new concept--urban and transportation planners, health professionals and advocacy groups have been working on these issues for decades.
- Flagler Partnership for Community Health
The Flagler Partnership for Community Health is a county-wide health assessment and planning initiative sponsored by the Flagler County Health Department.
- Partnership for a Healthier Nassau
Partnership for a Healthier Nassau (PHN) is Nassau County's community health improvement process. PHN is led by the Nassau County Health Department, in collaboration with other key community organizations.
- Clay County Collaborative
Community leaders from Clay County are working together to create a Clay County Collaborative that will improve safety-net services and accessibility within the community.
